Service Description

Provides support services to First Nations clients who seek services in the Prairie North Health Region. The Hospital Liaison Worker provides the following services:

  • Advocacy
  • Discharge Planning
  • Assistance with Forms
  • Referral
  • Attending Discharge Rounds
  • Supportive Services
  • Translation
  • Medical Transportation Assistance
  • Follow Up
  • Reassurance Visits
  • Long Term Care Visits

This program is a partnership between the Prairie North Regional Health Authority and the Battle River Treaty 6 Health Centre.

First Nations clients who seek services in the Prairie North Health Region can contact the Hospital Liaison Worker at 306-446-6600.
